Dear Madam / Sir,

Re: Walker v Heron Hotels Ltd.  Case No: 35240/LNA

I attach a copy of my letter to the respondent dated (date). The respondent has not provided the requested information and disclosure. I should be grateful if the tribunal would make an Order. An Order for the requested information and disclosure will assist the tribunal in dealing with the proceedings efficiently and fairly. The questions and documents are relevant in establishing the true reason why the claimant was dismissed and whether the reason was related to pregnancy.

I am writing to the respondent today in accordance with rule 11(4) enclosing a copy of this letter.

Yours faithfully,

(Claimant or claimant’s representative)
